Your First Visit

We are pleased to welcome you to our office as a new patient!

Please complete the Patient Registration Form prior to your first appointment. This form provides us with important health history and insurance information which allows us to properly prepare for your visit.

Also, please bring:

• your current eyeglasses,
• contact lens information,
• and, if available, prior eye exam records (including older prescriptions)

Please note, in some cases a dilated eye exam is necessary. A dilated eye exam requires placing a drop in each eye, which allows the pupil to remain open for a greater view of the retina. Please bring sunglasses and possibly someone to drive you home as your vision may be blurry.

Meet Ken.When you call to set up an appointment you may speak to him. Ken is my husband! He is an attorney with a legal practice and we share office space. After nearly ten years, he has gotten pretty great at adjusting glasses and picking awesome frames. Yes, we work together every single day!
