Your First Visit

We are pleased to welcome you to our office as a new patient!

Please complete the Health History Form below and return to our office at least 2 days prior to your first appointment. You can type your information into the form by clicking on the form fields or print and fill them in by hand. This form provides us with important health history and insurance information which allows us to properly prepare for your visit.

Also, please bring your current eyeglasses, contact lens information and, if available, prior eye exam records including older prescriptions.

Please note, in some cases a dilated eye exam is necessary. A dilated eye exam requires placing a drop in each eye, which allows the pupil to remain open for a greater view of the retina. Please bring sunglasses and possibly someone to drive you home as your vision may be blurry.

For the protection of your information, it should not permit you to save or send the data via email, but rather have you print the form, after which you can fax, mail or deliver to our office.
